OFFICE POLICY

  • We accept many insurance plans, including most private insurances, medicaid HMOs, and straight medicaid.
  • All copayments and balances are due at the time of visit.
  • Please bring your insurance card(s) & ID to every visit and a signed note of proxy if your child is being brought to the visit with someone other than their legal guardian.
  • Please arrive 10 minutes prior to your scheduled appointment.
  • All Brighton & Montclair office visits are by appointment only.
  • Weekend & Late Night well visits are by request only.
  • If you are unable to keep your appointment, please call at least 24 hours in advance to cancel or reschedule.
  • Missed Appointment/ No Show: If you miss an appointment and do not give the office sufficient notice, you are considered a no-show, which may be subject to fees. Full details are outlined in our financial policy and available in the patient registration packet.
  • Late For Appointment: If you are late for your appointment by 15 minutes or more, your appointment will be canceled and subject to a no-show fee as described above. Well visits will always have to be rescheduled.
  • If you are more than 15 minutes late for your child’s sick visit we can only accommodate you as the schedule allows. Every attempt will be made to have your child squeezed in amongst other patients. Wait time cannot be determined.
  • If you have a non-emergency question, please call us during office hours. We will return your call on the same day.
  • After office hours, please call office number for urgent matters only that cannot wait until the next business day.
  • In case of emergency, please call 911 or go to the nearest emergency room.
  • Our office takes privacy & HIPAA very seriously. Patients have the right to a copy of our office privacy policy. This is available in the patient registration and may be requested at any time from the front desk.
  • All patients have the right to request their medical records for any reason, but must fill out a medical release form. All medical record requests are subject to standard fees per page. Full details are outlined in our financial policy and available in the patient registration packet.

VACCINE POLICY

Our patients’ health has always been our first priority. We believe that vaccinating in accordance with the CDC and AAP recommendations is the best way to prevent disease in children.  We do not accept patients who do not wish to vaccinate following the established AAP immunization schedule. We understand that parents may have concerns about vaccines, and invite you to discuss them with our providers. We want you to be comfortable in the care you receive at NuHeights Pediatrics, and most importantly, we want our children to be healthy.
